OUR MEMORY CARE COMMUNITY

Select Senior Living offers memory care apartments with 24-hour Home Health Aides who assist dementia and Alzheimer’s residents, relieving them from daily activites that have become difficult to manage alone. Individuals are accepted into memory care following a comprehensive nursing assessment of cognitive, physical, and social levels of functioning. We provide a comfortable, home-style atmosphere and programs designed to stimulate memory preservation and development.

We design smaller group activities and programs that focus on each resident's capabilities as well as interactive games and group events for enjoyment. Our professional, compassionate associates engage our memory care residents in programs they can participate in, which enhances their sense of accomplishment and promotes their physical, emotional, and social well-being.

All of our home care team receive specialized memory care training that develops their understanding of and expertise in this field. We nurture a warm-hearted environment, and we are committed to making that experience last.

PERSONAL CARE PACKAGES

Assisted Memory Support  Resident cooperates in assisting with routine cares such as dressing, grooming, bathing, and toileting. Assistance is needed with medication monitoring, and resident’s behaviors may require occasional Home Health Aide intervention.

Moderate Memory Support — Resident may be incontinent of bowel and bladder. Resident requires direct assistance of one Home Health Aide with dressing, grooming, bathing, medications, and/or management of incontinence. Resident’s behaviors may require frequent Home Health Aide intervention.

Advanced Memory Support — Resident may be incontinent of bowel and bladder. Direct assistance from one Home Health Aide is required for dressing, grooming, bathing, medications, and/or management of incontinence. Resident’s behaviors may require frequent Home Health Aide intervention. Package includes moderate memory support and assistance of a two-person transfer.

  • Medication packages — Prices are based on number of visits per day. Appropriate for clients who have few care needs but need assistance with medication compliance.
  • Medication set-up — A nurse places medications in individualized, labeled containers for client. The service includes coordination with the physician and pharmacy for medication orders, compliance assessment, response to medications, and consultation with resident or responsible party as needed.
  • Medication change — A nurse will adjust the medications to comply with the revised medication order and reset the resident’s labeled containers.
  • Nurse visit — Nurse will provide services ordered by the physician and agreed to in the service plan. Nurse on-call for phone consultations during evenings, weekends, or holidays.
  • Home health aide visit — A home health aide will provide supportive services identified in your service plan.

FINANCIAL OPTIONS AVAILABLE

But oftentimes, financial constraints can hinder seniors from joining an assisted or independent living community. That’s why we proudly offer Minnesota’s Elderly and CADI Waiver options to help bridge the gap between individual income and cost of care while providing the resources and services necessary to thrive in our vibrant and nurturing environment.

ELDERLY WAIVER

Designed for individuals 65+, Minnesota’s Elderly Waiver option offers financial assistance to eligible seniors to help alleviate the financial burden associated with moving to an assisted living community. The program provides funds for home and community-based services for individuals with limited financial resources who require the level of care provided by a nursing home.

CADI WAIVER

Designed for individuals under the age of 65, Minnesota’s CADI Waiver option assists with the cost of services associated with Independent Living. The program provides support for eligible seniors who require nursing home care but enables them to continue living independently.
